Do I have to give this disclosure to a tenant every year, or only once when they move in?

In New York City the landlord has to give the tenants (in buildings with 3 or more apartments) a form to complete and return to the landlord before February 15, each year.

I give it to the tenant when they move in. MAke sure that they sign a copy saying they recieved it and keep it in your tenant's file. No need to give it to them every year.
